Champion Level weapons and armor can be synthesized from shards when all shards have been collected or won from Epic Chests (the type that costs diamonds.) A few Champion pieces are won as quest rewards or special event prizes.

Green and Blue armor/weapons is most commonly looted from regular gameplay and most often used to upgrade Purple, Orange and Champion armor/weapons.

The Champion weapons/armor comes in three sets: Hades, Neptune and Champion. Each has different attributes depending on the type.

Purple weapons/armor can be used to upgrade or smelted for Market Crystals which are redeemed for weapons, shards, Gold, Runes or Refinement Stones in the Market Shop.

Orange weapons/armor can also be smelted in the shop for a much higher reward, but hold on to them for a while. Once your character reaches a higher level, you will have extra spaces to equip more items for a Might/Damage/Health/Attribute boost.

Upgrading

Upgrading weapons and armor is accomplished via the upgrade tab in the inventory.

Upgrading can be accomplished by either automatically selecting pieces to be integrated into the armor/weapons to be equipped or manually selecting them.

If you run out of upgrading materials, you can either use a Purple or Orange item, loot a chest or use some Siderite. Upgrading with Siderite is only an option when you have no more available Green or Blue items. After your Blue or Green inventory is depleted, selecting the "Add Automatically" bar will toss either enough Siderite in to bump the Item being upgraded up one level or use the remainder of your supply.

Siderite can be purchased from the VIP Shop or the Market Shop. It can be looted in Survival Instances. It can be pickpocketed and synthesized. It can be found in chests. It is everywhere.

Upgrading and refining costs more Gold each time the item is upgraded/refined.

An item can only be upgraded to a certain point for each level a character reaches.

What you choose to upgrade your equipment with is almost completely irrelevant. The upgrade materials only contribute to the overall experience point system used in game to determine the item's fortification level and associated Health/Damage benefits.

Refinement Stones are used to increase an item's contributed Might with a slight health/damage benefit as well. There is a cap on the amount of refining that can be done per item per character level, just like upgrading.

Refining an item can only be done with Refinement Stones, so start collecting. Higher level equipment will start soaking up 400 stones an item for a good refining and there is no option to go halfway with refinement. You either have all 400 or no refinement for you.
