It means it takes up 2x2 space or 4 squares. Whereas, 1x1 decoration take up only 1 space.

2x2 refers to the number of empty spots required in order to place the decoration on your island. So a 2x2 decoration requires 4 empty spots, 2 wide by 2 long.
