How do you activate blood rage?

You can get blood Rage at Moon Realm ( Tokyo and Chizuru). You can find a black/red orb floating randomly around the map and loot it.

You can also increase your chance on activating it using the Drac effect RISK FACTOR

Else you may get it around once a month randomly

It happens randomly. But in story it happens because of plot

I dunno if it happens randomly because for me it only happens in story mode but you have to collect all the dracs to fully unlock it
