I can’t kill the hostage holder in al vahdeko even though it shows head shot. Why?

It's likely that your damage on the ammo isn't high enough. You have to shoot them twice if you haven't upgraded enough. I've done that many, many times. Have to be quick with the second shot. If you are quick enough, the animation will start and you'll complete the level.
