Where can I find the npc that trades for unique items?

I think he spawns randomly. But for me the most consistent way of finding him is at Chamber of the Exiled King at Painful or Fatal difficulty.

I found it once in Wild thickets, over the first structure, but never seen again there, probably changed in the last update.
